Downs Division of Solid and Hazardous Waste Department of Environmental Quality PO Box 144880 Salt Lake City, UT 84114-4880 RE: Follow-up Report for Emergency Permit #UT-037-2009 Dear Mr. Downs, In accordance with the requirements of Emergency Permit No. UT-037-2009, ATK Launch Systems, Promontory has prepared this report to summarize activities in the 1st quarter of 2010. The time frame for this report is January - March 2010 72 flares were launched for this quarter, all M257s. All de-mining activities authorized by this permit were performed on the KOSMO Test range. De-mlning activities successfully treated the flare(s) and verified by visual inspection. The remaining residue/debris was properly disposed in the ATK Promontory Landfill. Demining activities occurred as outlined below under the direction of Mr. Clif Oldham, Test Operations: Date Flare type Flares demined Time Location 13 Jan 2010 M-257 1 0840 hrs N.41°42'32.3" W. 112°47'39.4" Three flares launched in March failed to separate and are embedded in the mud. The locations are confirmed and will be recovered with a backhoe when conditions permit.
