Subject: Construction Certification for RD & D Permit
Enclosed are copies of certifications from professional
engineers dealing with the design and construction of the
Solid Rocket Motor Disposal Facility (SRMD) along with the
red-lined drawings. This is being submitted in accordance to
the RD fit D Permit, Module II, part B. Enclosed is a copy of
the certification for both the design of the SCWO tank system
and construction.
Please schedule an on-site visit at your earliest convenience
to verify the design and construction. Your help in this
matter is greatly appreciated.

CERTIFICATE OF DESIGN ASSESSMENT
I, Nnrman T. Bennion. P.E., have perfbrmed a design assessment for a portion of a new tank
system for the Solid Rocket Motor Disposal Fadlity (SRMD), located at ThiokDl's Plant near
I^omontory Point, Utah, owned/operated by General Atomics. My duties were to attest tfae
adequacy of the SCWO Reactor Skid Framework design, as shown on tbe drawings listed
below, to satisfy seismic Zone m requirements as required by tfae Resource Conseivation and
Recoveiy Act (RCRA) reguladons, namely 40 CFR 264.192, paragraphs (a)(5)(ii).
The SCWO Reactor Skid and its framework, as designed, are capable of supporting all of die
reactor components during a seismic zone UI evem without structural fiulure of tfae
framework, and without overtuniing or sliding of tbe skid.

Project Equipment Installation Walkdown
In accordance with your request, I have performed a walkdown of
the SRMD equipment. The objectives of the walkdown were to
verify that all major components and tank systems shown on the
drawings were installed; installation was generally in
accordance with the drawings, and that good engineering,
construction and installation practices were generally
followed. The walkdown took approximately 11/2 hours and was
performed on January 16, 1995 against the following drawings:
620024 - SRMD PRS Equipment Arrangement, Sheets 2, 3 & 4
Rev C (See attached red lines)
620106 - HTO Equipment Arrangement, Sheet 1 of 1, Rev A
(See attached red line)
620501 - SRMD Hydrolysis, Layout, Sheet 1 of 1, Rev N/C
(See attached red line)
The walkdown verified that all the equipment was installed and
appeared ready for operation except as noted below. The
equipment design appeared to be consistent with good/standard
engineering practices. The installation appeared to be
consistent with standard construction practices. No obvious
deficiencies were observed. The walkdown was limited to visual
inspections consistent with the level of detail shown on the
referenced drawings. No measurements were made and operational
performance was not verified.
The following minor deviations from the referenced drawings
were noted during the walkdown:
620024, Sheet 4 of 4: Major equipment not shown on drawing:
1. Warm air ducts installed along the north side of the motor
washout fixture.
2. LNj boost pump (adjacent to high pressure pump).
3. Electrical panel/junction box adjacent to LNj tank #1.
4. Two LNj vaporizers adjacent to LNj tanks.

620106, Sheet 1 of 1:
1. Equipment pad east of HTO building. The following
equipment was not shown on the drawing:
A. Pump adjacent to TM-3000
B. Valve panel in center of pad
C. Vaporizer near valve panel
2. Inside HTO Building:
A. Minor plumbing and valve connections incomplete
throughout
B. Air compressor not anchor bolted to floor and filter
at discharge was not well supported
C. Flow meter (FE-200) not anchor bolted to floor
D. Pump P-700 inside building rather than outside as
indicated by redlines
E. Vertical reactor was not installed on reactor skid
F. Polycarbonate panels not installed around reactor
skid.
620501, Sheet 1 of 1: Major equipment not in place:
1. Pump P-2: not permanently installed on pads. A pump
presently off the pad in use.
2. Hydrolysis tank off to the side on flat bed truck.
3. 600 gallon SCWO feed/transport tank not on site; to
be same as horizontal plastic tank in place as T-5
per site engineer.
4. Tank T-5 is plastic horizontal tank rather than
vertical cylindrical tank as shown on drawings.
