RE: Follow-up Report for Emergency Report #UT-027-2009
Dear Mr. Downs,
In accordance with the requirements of Emergency Permit No. UT-027-2009, ATK Launch
Systems, Promontory has prepared this report to summarize activities in the 4'^ quarter of 2009.
The time frame for this report is October - December 2009.
All de-mining activities authorized by this permit were performed on the KOSMO Test range.
De-mining activities successfully treated the flare(s) and verified by visual inspection. The
remaining.residue/debris was properly disposed in the ATK Promontory Landfill.
Demining activities occurred as outlined below under the direction of Mr. Clif Oldham, Test
