Box 144880 ^OlD- 00^^ Salt Lake City, Utah 84114-4880 Attention: Jeff Vandel Re: ATK Launch Systems-Promontory EPA ID number UTD009081357 Hazardous Waste Storage Permit September 2008 Email Notification to Mr. Jeff Vandel January 08, 2010 Dear Mr.Downs: On January 8, 2010 notification was given by email to Mr. Jeff Vandel of a spill containing perchlorate. Our Hazardous Waste Storage Permit in Module I, I.T.5. requires that a report be submitted within 15 days. During the month of December, a culinary water line and a fire suppression line containing culinary water, fi-oze and broke in a building that is currently not in use. The onsite fire department responded to the alarm and shutoff the water. The water had spilled onto the floor and then flowed into a sump. A small amount of water flowed out ofthe door onto the asphalt forming a small pool of frozen ice. This frozen pool was sampled and found to contain a perchlorate level of 24.6 mg/L. Judging from the size ofthe frozen pool on the asphalt it did not appear to be a large quantity of water released. Following the notification on January 8^*^ a soil sample was collected and analyzed from a drainage ditch located just off of the asphalt pad. This sample reported a perchlorate level of 2.2 mg/Kg. This level is below the residential Regional Screening Level (RSL) of 55 mg/Kg. It does not appear that the water released from the building resulted in a large quantity of water released to the environment. This small pool of ice is still frozen and efforts will be made to chip it up and remove the contamination and then treat it at our M-705 facility.
