Box 144880 Salt Lake City, Utah 84114-4880 Dear Mr. Downs: Subject: T-15 Daily Inspection Building T-15 is a permitted facility under the RD & D Permit issued to Thiokol Corporation. This letter is notification that on 11 January 1995, the daily inspection was not conducted. The daily inspections at T-15 are handled by the security force as required. On 11 January 1995, two security guards were in a car wreck when leaving the plant. They were both taken to the hospital for evaluation. Other members of the security force responded to the scene and were involved that morning. Due to the impact of that event on their organization, the daily inspection at T-15 was missed. On 13 January, when verifying inspection completions, it was noted that the weekly inspection for M-22 5 Burn Grounds was not conducted the week of 25 to 31 December 1994. Related inspections were completed for the 90-day docks in the area, but due to a misunderstanding, the Burning Grounds area was not inspected. There was not any waste or operating activity at M-2 25 during that week. Waste Management and Environmental Services is working to take corrective action on these situations.
