Subject: ATK Thiokol Propulsion (EPA ED # UTD009081357) Closure of Waste Motor Storage Buildings M-l07, M-340 & S-616 As authorized by The Division ofSolid and Hazardous Waste in yoiu- October 27, 2003 letter, we have completed our closure activities for buildings M-l07, M-340 and S-616. In compliance with our approved closure plan, we have removed all waste propellant/motors, and cleaned and inspected the floors for signs ofcontamination. Our inspection has demonstrated that the buildings are clean and free of hazardous contaminants. We have also contracted with an independent registered professional engineer to certify that all waste has been removed, and that there are no signs ofcontamination remaining in these buildings. A copy ofthis certification is enclosed for your review. Please contact Gene Curtis at (435) 863-4479, ifyou would like to make arrangements to inspect these buildings prior to acceptance of closure. Upon Division acceptance of these closures, we will revise and update our closure plan and submit to the State for approval. I certify imder penalty oflaw that this docimient and all attachments were prepared under my direction or supervision in accordance with a system design to assure that qualified personnel properly gather and evaluate the information submitted. Based on my inquiry ofthe person or persons who manage the system, or those persons directly responsible for gathering the information, the information submitted is, to the best ofmy knowledge and belief, true, accurate and complete. Box 689 Mail Stop 301 Brigham City, UT 84302-0689 Subject: Closure Certification for M-l07, M-340, and S-616 Dear Mr. Curtis: In accordance with closure plans approved by the Utah Division ofSolid and Hazardous Waste, 1 conducted an independent inspection of buildings M-l 07, M-340, and S-616 on January 20, 2004. The purpose ofthis inspection was to verify that all reactive waste had been removed from the buildings and that no visible signs of propellant contamination remain. This evaluation included a visual inspection ofthe floor in each building looking for potential cont£imination. Based on my inspection, I hereby certify that all reactive waste has been removed from the subject buildings and that no visible signs of propellant contamination remain. It is, therefore, my opinion that the buildings have been properly closed in accordance with the respective closure permits. I certify under penalty of law that this document and all attachments were prepared under my direction or supervision according to a system designed to assure that qualified personnel properly gather and evaluate the information submitted.
