Re: Signatory Requirements
Dear Mr. Thompson:
Permit Con(Jition I.BB. of the Thiokol Part B Storage Permit requires that all submittals to the
Division regarding this permit meet specific signatory and certification requirements. These
requirements are outlined in R315-3-8 ofthe Utah Administrative Code (the Rules).
It has come to our attention that Thiokol Corporation has not been following all of these
requirements, including the certification statement that must accompany all reports and
information conceming the permit. In addition, these regulations require that only designated
persons sign and certify submittals. Thiokol is requested, within 30 days of receipi of this letter,
to submit a list of all members of its staff that fulfill the requirements of R315-3-8 and are duly
authorized to be signatories on permit correspondence. All submittals subsequent to that date will
require the certification statement.
