Box 707 Brigham City, Utah 84302-0707 Re: ATK Thiokol - Promontory Emergency Permit UT-033-2006 UTD001705029 Dear Mr. Gooch: Attached is Emergency Permit UT-033-2006, authorizing ATK - Promontory to treat, by open burning, approximately 1000 pounds of uncured LUU-19 illuminant that was accidentally spilled on the ground at the M-136 burn grounds. This emergency treatment permit was in effect for May 25, 2006. The U.S. Environmental Protection Agency (U.S. EPA) has authorized the Executive Secretary to issue such a permit under Section 3006(b) of the Resource Conservation and Recovery Act (RCRA). This Permit is issued under the authority of 19-6-101 through 105, Utah Code Annotated, as amended and R315-3.6.2. Also attached to this letter is a copy of the newspaper notice and the Emergency Permit Request Form. ATK - Promontory is required to submit a copy of the burn log for May 25, 2006 to this office on or before June 22, 2006. The burn log should clearly indicate the burn station where the treatment of this waste occurred. If you have any questions, please call Bill Wallner at (801) 538-6170.
