Dear Mr. Hancock:

This letter is in response to Thiokol’s Monitoring Well Installation Plan for two additional “J – series” wells that was submitted to the Division of Solid and Hazardous Waste (the Division)
on August 4, 2004. This Plan was submitted pursuant to Post-Closure Permit Condition V.D.2.b. This condition states that the installation of new monitoring wells shall be done in accordance
with plans and specifications to be submitted to the Executive Secretary for approval.

The Monitoring Well Installation Plan is hereby approved. Please be reminded that a Monitoring Well Completion Report for the J-series wells needs to be submitted to the Division within
90 days of completing well installations, as indicated in Permit Condition V.D.2.e. This report needs to include geologic logs, potentiometric surface data, well construction logs or
diagrams, well development results, slug or pump test results, well survey data and a summary of the results of each monitoring well installation (e.g., was the well successfully installed,
were hydrogeologic conditions as expected, etc.).
