Subject: Submittal of M-136 LTTA Post-Closure Permit Annual Report

Enclosed is the submittal of the M-136 Liquid Thermal Treatment Area (LTTA) post-closure
permit annual report. The post-closure permit for the M-136 LTTA, Sections V.F.3 and V.F.4
require semiannual of reports detailing results of sample analyses including concentration of
hazardous constituents and a simimary of QA/QC data, measurements of static water levels,
and total well depth estimates.
Thiokol was unable to sample compliance well A-4 during 2002 as required by the post-
closure permit because of technical difficulties. During repair of the well in Spring 2001, a
PVC access pipe was dropped, jamming the purge pump to the side of the well. We obtained
the service of a well repair company to remove and repair the pumps, but this effort was
unsuccessful. We hope to modify this sampling requirement when the Post-Closure permit is
renewed this Fall.
