Baily, M.D., M.S.P.H., Bear River Health Department NOTICE An emergency hazardous waste treatment permit UT-21-98 was issued to the Thiokol Corporation pursuant to RSI 5-3-19 ofthe Utah Administrative Code. The permit provided Thiokol authorization to treat unstable flares by open burning at their Promontory facility from May 5, to May 8, 1998. The flares were rendered unstable as the result of incomplete burning during a test procedure, it was determined to be unsafe to collect the flares and transport them to the open burning facility, therefore the Einergency Permit was necessary so that the waste could be treated in place. The permit was issued by the Utah Department of Environmental Quality, Division of Solid and Hazardous Waste. Copies ofthe emergency permit are available for public inspection during normal business hours on the 4th floor ofthe Martha Hughes Cannon Health Building, 288 North 1460 West, Salt Lake City, Utah.
