Box 689 Brigham City, UT 84302-0689 CERTIFIED MAIL RETURN RECEIPT REOUESTED SUBJECT: Utah Hazardous Waste Emergency Permit UT-40-97 Issued to the Thiokol Corporation Dear Mr. Curtis: Enclosed is Utah Hazardous Waste Emergency Permit #UT-40-l 997 providing Thiokol authorization for storage of 20 drums and 30 boxes of class I.ID explosive waste and contaminated debris. It is anticipated that the explosive waste will be stored as described in your letter requesting the permit, and will be treated by September 16, 1997. This permit became effective on September 12, 1997, and will expire on September 16, 1997. The pennit was issued under the authority of 19-6-101 through 105, Utah Code Annotated, as amended and R315- 3.19(a). Bailey, M.D., M.S.P.H., Health Officer/Department Director Bear District Health Department, w/enclosure Kathy Barker, DSHW September 16, 1997 STATE OF UTAH EMERGENCY PLAN APPROVAL UT-40-97 Permittee: Thiokol Corporation Brigham City, Utah Pursuant to the Utah Solid and Hazardous Waste Act, (the Act), 19.6.101-122, et. seq., Utah Code Annotated as amended and the Utah Administrative Code (UAC) (R315-1 through R315-13 and R315- 50) as adopted by the Utah Solid and Hazardous Waste Control Board, an emergency plan approval (herein after called "permit") is issued to the Thiokol Corporation (hereafter called the "Permittee"), to store 20 dmms and 30 boxes of class I.ID explosive waste. The emergency storage permit was requested by the Permittee to allow time for safe handling procedures to be developed prior to treatment. The U.S. Environmental Protection Agency has authorized the Executive Secretary to issue such a permit under Section 3006(b) of the Resource Conservation and Recovery Act (RCRA). The Permittee must comply with all the terms and conditions of this permit. The permit consists of the applicable portions of R315-1 through R315-13, R315-50, and R315-101 incorporated by reference. Applicable rules are those which are in effect on the date of issuance of this permit. This permit is based on the premise that the information provided to the Executive Secretary on September 11,1997 is accurate and that the storage will be conducted as specified in the Permittee's letter requesting the permit. Any inaccuracies or misrepresentations found in the application may be grounds for the termination of this permit (see R315-3-19). The Permittee must inform the Executive Secretary of any deviation from, or changes in the information in the application which would affect the Permittee's ability to comply with the applicable regulations. This permit is effective as of September 12, 1997 and shall expire on September 16, 1997 unless terminated in accordance with R315r3-19.
