As you are aware, most ofthese problems were corrected shortly after the facility inspection. We believe items 1 - 4 mentioned in your letter have been adequately addressed and corrected. The steps Thiokol has taken to correct each item are outlined below. First - Inspection forms are now being reviewed and signed prior to filing . This will assure that all problems are corrected and noted on the inspection form in a timely manner. Thiokol will also submit a revised T-15 inspection form for State approval with our RD&D permit renewal request. Second - A tabular data base has been created which lists all Dock Coordinators, date assigned to hazardous waste duties and date first trained. Training records for all emergency coordinators, including Ray Stokes, are kept in our computer tracking system which is accessible plant wide. These individuals are not typical hazardous waste handlers, but are required to complete HAZWOPER Incident Commander training. Hazardous waste specialists are available to the Emergency Commanders to assist them in proper management ofany waste from a cleanup. Third - A copy ofthe contingency plan has been placed in M-705 south. Fourth - The Part A has been updated and is included with this letter.
