SUBJECT: Response to UDEQ 20 February 1996 Letter-Thiokol 1995
Inspection: T-15 Inspection Form Modification
In response to your above referenced letter, Thiokol submitted
information on 7 March 1996, to your office related to the 1995
inspection performed by the UDSHW at the Thiokol facility. An
example of the modified inspection form for the T-15 permitted
storage facility was inadvertently not included in the
submittal. A copy of the new form was reviewed with Otis
Willoughby when he was here at Utah DLV Operations recently.
A copy of the modified inspection form currently being used at
T-15 is enclosed.
