Box 689 Brigham City, UT 84302-0689 Re: 1995 RCRA Inspection Dear Mr. Thompson: On October 17-23, 1995, the Division of Solid and Hazardous Waste conducted the annual Compliance Evaluation Inspection for hazardous waste management at Thiokol. The inspection team met with members of the Thiokol Environmental Office and then proceeded to visit many hazardous waste generation and storage sites at the Thiokol Plant. The inspection also included a review of the hazardous waste operating record. The Division is generally pleased with the overall management of hazardous waste at Thiokol. There are however, several areas of concem that the Division would like to see addressed; these are the same concems discussed at the close ofthe inspection on October 23. First, the daily inspection records for permitted storage at T-15 did not record that any corrective action had taken place when warranted. Thiokol has already taken a step to correct this problem by requiring a sign-off before the form is filed. Thiokol should take further action by modifying the inspection form to include an area for a description of the corrective action, date action was performed, and people responsible for the action. Second, the training records for the employees (past and present) that work in hazardous waste management should be filed in such a manner that the inspectors can easily access them. The records for past employees should be kept on hand for three years. It took several days for the records that were requested to be recovered from archives. Also, the training records for the Emergency Coordinator, Ray Stokes, should be kept with the other records for hazardous waste managers, not only at the Fire Department as is presently done. February 20, 1996 Page 2 Third, a copy ofthe Contingency Plan shall be placed in the permitted storage area at M-705. Fourth, the Part A permit for storage at Thiokol is out-of-date. This document still lists Jack Martin as the Facility Contact. Thiokol should thoroughly review the Part A, and bring it up to date with current personnel and hazardous waste management activities. Within thirty (30) days of receipt of this letter, the Division would like to receive, from Thiokol Corporation, a response to this letter outlining corrective measures taken to address the problems noted above.
