SUBJECT: Class 3 Permit Modification (94-062) to Thiokol Hazardous Waste Storage Permit (UTD
009081357).
Thiokol's request for a permit modification has been approved as requested in your April 25, 1994
letter, Part B Permit Modification and Fee Transmittal, EPA ID UTD009081357. This is a Class
3 modification to the Thiokol Permit. All other permit conditions remain in effect with regard to
complying with the rules in the Utah Administrative Code R315.
Copies of the modified pages to the permit are enclosed. Please replace the corresponding pages in
your copies of the permit with these new pages.

NOTICE
Thiokol Corporation has submitted a Risk Assessment Report and Site Management Plan for in-
place closure of two industrial wastewater ponds located at M-186. A Risk Assessment was
performed by Thiokol which found that the risk for exposure or harm to human health and the
environment was within acceptable limits. The Executive Secretary of the Utah Solid and
Hazardous Waste Control Board agrees with the risk assessment findings and has granted draft
approval of the assessment report.
The public comment period for the Site Management Plan began September 6, 1995 and will end
October 5, 1995. A public hearing concerning the plan has been scheduled Monday, October
2, 1995, at 7:00 p.m. in the auditorium ofthe Brigham City Library.
