Bailey, M.D., M.S.P.H., Health Officer/Department Director Bear River District Health Department Printed on recycled paper TITAM '^UM-mi^ INFORMATIONAL NOTICE ONLY A Class 3 pennit modification request was submitted to the Division of Solid and'Hazardous Waste for the Thiokol Facility in Box Elder County, Utah to incorporate the following changes into the current Thiokol permit: Change 1 - Attachment 13 - Modifies the procedure for the dilution of Hydrazine with water to allow a final concentration of five percent to fifty percent by volume of hydrazine. Change 2 - Module II, General Facility Conditions and Attachment 3 - Changes the yearly training required for "on board" personnel to reflect current needs. Change 3 - Module III, Containers - Adds additional "D", "P", and "U" waste codes to the ones currently authorized. Change 4 - Module III, Containers - Allows the storage of solids that do not contain free liquids in gondolas. Change 5 - Attachment 1 - Modifies the Waste Analysis Plan and Thiokol Environmental Laboratory Quality Assurance Plan to incorporate the latest technology (Such as specifying analytical procedures from SW-846 issued July 1992), and incorporating the latest information on topics such as waste stream analysis requirements. The initial public comment period for this request was held from February 22 to April 24, 1995. A public meeting was conducted by Thiokol on March 22, 1995. The staff of the Division of Solid and Hazardous Waste reviewed the proposed changes and found them to be acceptable. A draft approval to incorporate the changes mto the Thiokol permit, along with draft changes to the Thiokol permit to ensure consistency with the permit has been prepared and is available for review during the public comment period. Public comment on the draft decision to revise the Thiokol permit and incoiporate the changes into the permit will commence on May 15, 199^, and conclude at 5:00 pm on June 29, 1995. Public hearings on this draft approval will be held on Wednesday, June 14. 1995. at the Brigham City Library, 26 East Forest, Brigham City, Utah at 7:00 pm, and on Thursday, June 15, 1995, at the Cannon Health Building, 288 North 1460 West, Salt Lake City, Utah, room 406 at 10:00 am. The draft changes to the Thiokol permit are available for review during the public comment period beginning May 11, 1995 between the hours of 8:00 am and 5:00 pm, Monday through Friday at the offices of the: Division of Solid and Hazardous Waste 4th Floor, Martha Hughes Cannon Building 288 North 1460 West Salt Lake City, Utah Additionally, a copy of the draft changes to the permit are available for review during the public comment period at the following locations: Brigham City Library Logan City Library 26 East Forest and 225 North Main Brigham City, Utah Logan, Utah Written comments on the draft approval must be received no later than 5:00 pm on June 29, 1995 and should be submitted to: Dennis R.
