Box 144880 Salt Lake City, Utah 84114-4880 Dear Mr. Downs, Subject: Missed Inspection at Thiokol's RD&D Permitted Facility T-93 Thiokol failed to complete a required inspection of the T-93 permitted facility on February 4, 1995, The Department of Environmental Quality was notified by telephone upon discovery of this missed inspection. This letter is the formal written notification to the State as required under permit condition LS. of our RD&D permit. This permit requirement deals primarily with spills. Many of the information items requested in this section of the permit do not apply to a missed inspection and are not included in this letter. Thiokol's RD&D permit requires a daily inspection whenever hazardous waste is stored at this facility. The personnel that run this facility generally work Monday through Friday and are not available for weekend inspections. In order to comply with this permit requirement, we have trained our Security department to perform this function on weekends and holidays. Although the importance of this inspection was stressed during training of the crew chiefs, it apparently was not completely effective in their training of the guards. When a guard arrived on February 4 for inspection, the gate was locked and he did not have the lock combination. Because of his lack of understanding of the importance of this inspection, he left without completing the inspection and did not inform the crew chief until late that night. Downs 15 February 1995 Page 2 In an effort to prevent this from happening in the future, Thiokol has taken the following steps: 1. A memo has been sent to all guards explaining the importance and urgency of these inspections 2. A common lock to which all guards have a key has been placed on the access gate to the T-93/T-94 area 3. Home phone numbers of several key personnel have been given to Security, with instructions to contact one of them if there is ever a problem with completing an inspection or if a problem is discovered during an inspection We believe that these actions will greatly reduce the possibility of missed inspections in the future. "I certify under penalty of law that this document and all attachments were prepared under my direction or supervision in accordance with a system design to assure that qualified personnel properly gather and evaluate the information submitted. Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering the information, the information submitted is, to the best of my knowledge and belief, true, accurate, and complete.
