Dear Mayor Mike Farrar:The Division of Drinking Water (the Division) received an amended request for the Operating Permit for the New Cooke Well from Nathan Wallentine, Sunrise Engineering
on May 30, 2024. Previously, In November of 2006, the well was drilled, and the surface seal was witnessed. Well-equipping plans and specifications were submitted to the Division in
June 2007. Review comments were sent to you on August 17, 2007. Plan approval for equipping the well was issued on August 18, 2008.Project HistoryOn January 24, 2011, the Division of
Drinking Water (the Division) received a request for an operating permit (OP) for the New Cooke Well (previously known as Well No. 4) from Northern Engineering. At that time, an operating
permit could not be issued because the land use agreements had not been recorded with the county. Upon receiving that operating permit request, it was communicated to the water system
that had to be completed before an OP could be issued. After realizing that this source had been in use for many years and that the requirement for land use agreements/coverage under
the Washington County Source Protection Ordinance had not been completed, a final Drinking Water Source Protection (DWSP) Plan was submitted in December 2023. After a series of comments
and revisions, the DWSP plan was concurred with on May 30, 2024.
Our understanding of the well is that it is an 8-inch well drilled to a depth of about 300 feet. It is currently equipped with a 15 HP Gould Model 65L submersible pump. The pump is rated
to pump 70 gallons per minute. The well discharge piping and necessary appurtenant valving is housed in a well house. The New Cooke Well is identified as WS004 in the Division’s database.
We have received the following information for New Cooke Well (WS004):
Design engineer’s statement of conformance with approval conditions and a site visit to verify these conditions
Documentation of valid water right.
Evidence of O&M manual delivery.
As-built or record drawings.
Documentation that the requirements for coverage under the Washington County source protection ordinance have been met and has been verified with the concurred DWSP plan on May 30, 2024.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
We have determined that all conditions of operating permit issuance have been met. On this basis, an Operating Permit for the New Cooke Well is hereby issued as constituted by this letter.
Please maintain a copy of this letter with your permanent records for future reference.
The equipped well pump capacity of New Cooke Well is 70 gallons per minute (gpm). The safe yield of New Cooke Well is rated at 108 gpm, which is calculated based on two-thirds of the
constant-rate aquifer drawdown test results at 162 gpm. The safe yield of 108 gpm is the basis for determining the maximum number of connections that New Cooke Well can serve. In this
case, even though this well’s safe yield is calculated to be 108 gpm, the equipped pump capacity of 70 gpm is the physical limiting factor of well capacity.Routine Monitoring RequirementsThe
monitoring requirements for the New Cooke Well have been assigned and are given in the enclosed water monitoring report. Additionally, they are available to view anytime online at waterlink.utah.gov.

requirements for this system. The issuance of this Operating Permit resolves your water system’s deficiency S001 for 200 points related to having an “Unapproved Source in Service”. An
