Southwest Utah District Engineer Department of Environmental Quality 620 S 400 E #400 St. George, UT 84770 RE: Request Operating Permit for Kolob Heights Well (WS001), Well House, Transmission Line, and Tank – System #11831, File #12585, 12586, and 13095 Dear Paul, The Kolob Heights Well (WS001), well house, transmission waterline, and Tank were constructed according to the approved plans and specifications and any changes made were done in accordance with Utah Drinking Water Rules as described in the Utah Administrative Code, Rule Series 500. Changes that are noted include adjustments to the booster pump setup and sizing. These adjustments were done primarily due to permanent power source available. A booster pump skid sized and tested to maintain 98 gallons per minute at 60 pounds per square inch pressure was installed to maintain average and peak day flow demands. In coordination with WCWCD a third pump is going to be added to the system to provide another 49 gpm to the system to supplement peak times. The fire pump was placed on a variable frequency drive and set points to provide water during peak times when the smaller jockey pumps are unavailable to provide sufficient flow and pressure and for when a fire demand is required. Coordination was done with the local fire authority and fire chief to agree the use of the fire pump as a backup booster pump. A letter from Fire Chief Jim Banks from the New Harmony Fire Department is attached approving the use of the fire pump. During the plan approval period, waterlines were added but have not been design for the Jim Carter Subdivision that sits south of the highway and be connected to the system. Nothing has been constructed outside of Kolob Heights and plans will need to be reviewed and approved prior to adding these connections to the system. www.ensigneng.com Page 2 The operation manuals for valves and other appurtenances have been delivered to the owner, along with as-builts of the project.
