Subject:After-the-Fact (ATF) PA/Operating Permit, Well Equipping, Rollins Well #1 (WS001); Mountain Green Mutual Water Company, System #15046, File #13417
Dear Rulon Gardner:The Division of Drinking Water (the Division) received a Project Notification Form for equipping Rollins #1 Well from you on June 23, 2023. The Division issued a Plan
Approval for drilling Rollins #1 Well on February 28, 2007. Additional information including plans and specifications was received on August 10, 2023. A construction report for the well
drilling was also received on August 17, 2023. Division staff conducted an onsite inspection of these facilities on March 15, 2024, and confirmed that the well equipping has already
been completed. As such, this will be considered an After-the-Fact well equipping Plan Approval along with the Operating Permit.After-the-Fact Well EquippingOur understanding of the
project is the equipping of the existing well with a 10 HP Grundfos Model 85S100-9 90 gallon per minute (gpm) pump. A pitless adapter with a 4-inch diameter flanged connection was utilized.
The well was drilled to a depth of 160 feet with a 10-inch diameter steel casing extending 18 inches above grade to 132 feet below grade. The 10-inch diameter type 304 wire wrapped slotted
screen extends from 132 feet to 153 feet with a ¼-inch gravel pack from 127 feet to 160 feet. A 24-inch diameter steel casing was installed from grade to 14 feet deep. The cement grout
seal extends from 0 to 124 feet with a bentonite grout seal from 124 feet to 127 feet. A 20-inch diameter conductor casing was installed and cemented in place from 0 feet to 14 feet.
The 24-hour pump test identified a well yield of 90 gpm. This well is identified as WS001 in the Division’s database.This well was originally drilled in 2007 to serve as a new source
for Cottonwood Mutual (System #15013) but was never equipped. The ownership of this well has been moved from Cottonwood Mutual to Mountain Green Mutual Water Company.
We have received the following information for Rollins #1 Well Equipping:
Certification of well seal.
Well driller’s report (well log).
Pump test report.
New Source Chemical Analyses of the well water.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
Plans and specifications for equipping the well.
10 HP Grundfos Model 85S100-9 90 gpm pump.
4” C900 DR-18 PVC well discharge piping.
Baker Manufacturing monitor standard pitless adapter unit.
We have completed our review of the plans and specifications, stamped, and signed by W. Lee Watkins, P.E. of Hansen & Associates, Inc, dated August 5, 2008, and find they basically comply
with the applicable portions of Utah’s Administrative Rules for Public Drinking Water Systems in R309. On this basis, the plans for equipping Rollins Well #1 and well house are hereby
approved.
We have determined that all conditions of operating permit issuance have been met. On this basis, an Operating Permit for Rollins Well #1 and well house. You may now place Rollins Well
#1 and well house (WS001) in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
The equipped well pump capacity of Rollins Well #1 is 90 gallons per minute (gpm). The safe yield of Rollins Well #1 is rated at 60 gpm, which is calculated based on two-thirds of the
constant-rate aquifer drawdown test results at 90 gpm. The safe yield of 60 gpm is the basis for determining the maximum number of connections that Rollins Well #1 can serve.
Drinking Water Source Protection Requirement
The Division concurred with a Preliminary Evaluation Report (PER) for this source in February 2007. The Division has also received recorded land use agreements. A full Drinking Water
Source Protection (DWSP) plan is required within 180 days of the date of this letter. If the Division does not receive the DWSP plan within 180 days, a deficiency (SP07 - ACTIVE SOURCE
LACKS AN APPROVED DWSP PLAN) will be assessed against your Improvement Priority System report for failure to comply with this requirement. If you have questions about the source protection
