changes are deemed necessary and, based on review of the information provided, are hereby approved. An approved copy of change order is returned herewith for your records.The change
order No. 5addresses:1. During the installation of the 21” sewer gravity main in SR-165, unstable soilconditions adjacent to the trench excavation were encountered. As a result of these
conditions,some of the pavement adjacent to the trench separated in construction seams. This required theremoval and replacement of additional pavement by UDOT. Estimated Quantity overrun
1,443 SY.2. During the construction of the 21” sewer gravity main in 2900 South, it was requiredby the City of Nibley to provide Granular Backfill Borrow in the sewer trench in place
of nativematerial for the section of the trench between the pipe zone and asphalt pavement sections. Theoriginal bid was based on native material being placed in this section of the
trench. Additionally,during the construction and installation of the 21” sewer gravity main in 2900 South, the nativesoils adjacent to the trench excavation sloughed off into the trench
during excavation. Because ofthis, the estimated amount of Granular Backfill Borrow required for this work increasedsignificantly. Reference Work Change Directive #14.3. Due to the trench
sloughing and the poor condition of the asphalt pavement on 2900 South, itnecessary to replace all the asphalt pavement in 2900 South west of SMH M10 to SR-165. The original bid only
included asphalt pavement replacement for the sewer trench excavation.Estimated Quantity overrun 1,213 SY.This change order is for a contract increase of $238,523.18bringing the total
