Page 1 of 19 CHANGE ORDER NO.: 5 The Contract is modified as follows upon execution of this Change Order: Description: In compliance with EJCDC C-700, Article 13.03.E.1.(a)(b) and EJCDC C-700, Article 5.04.A, Article 5.04.D, and Article 5.04.E.3 the purpose of this Change Order is to incorporate the following changes into the Bid Documents: 1. During the construction and installation of the 21” sewer gravity main in SR-165, unstable soil conditions adjacent to the trench excavation were encountered. As a result of these conditions, some of the pavement adjacent to the trench separated in construction seams. This required the removal and replacement of additional pavement by UDOT. Estimated Quantity overrun 1,443 SY. 2.During the construction and installation of the 21” sewer gravity main in 2900 South, it was required by the City of Nibley to provide Granular Backfill Borrow in the sewer trench in place of native material for the section of the trench between the pipe zone and asphalt pavement sections. The original bid was based on native material being placed in this section of the trench. Additionally, during the construction and installation of the 21” sewer gravity main in 2900 South, the native soils adjacent to the trench excavation sloughed off into the trench during excavation. Because of this, the estimated amount of Granular Backfill Borrow required for this work increased significantly. Reference Work Change Directive #14. 3.Due to the trench sloughing and the poor condition of the asphalt pavement on 2900 South, it became necessary to replace all the asphalt pavement in 2900 South west of SMH M10 to SR- 165.The original bid only included asphalt pavement replacement for the sewer trench excavation. Estimated Quantity overrun 1,213 SY.
