I (We) acknowledge that I (We) own the property described above, and grant
permission to employees, contractor and subcontractor personnel, and other
representatives of the U.S. Department of Energy and the U.S. Environmental
Protection Agency to enter upon the property at reasonable time or times during the
next 36 months to conduct radiation surveys to determine the nature and extent of
any radioactive material that might be present. In addition, permission is given
to perform engineering assessments, if necessary, to evaluate the remedial measures
that might be taken, as well as to evaluate the extent of the work required and the
cost.
I (We) understand that DOE's responsibility for any damage or disturbance to my
(our) property caused by its activities shall be any backfilling, seeding, sodding,
landscaping, rebuilding or repair of the property required to restore it to a
condition comparable to its apparent physical condition immediately prior to entry
upon the property.
I (We) understand that the Monticello Vicinity Properties are a CERCLA (Superfund)
National Priority Listed (NPL) site and the cleanup of contaminated properties in
excess of EPA standards is mandated by law.

Report of Radiological Survey
1.0 Introduction
A radiological screening survey of DOE ID Number MS-01073 was conducted by
RUST Geotech Inc. (Geotech). This survey was conducted using methods, as defined in
the Vicinity Properties Management and Implementation Manual, UMTRA-DOE/
AL-05O601 (March 1988), the U.S. Department of Energy (DOE) Guidelines for
Residual Radioactive Material at Formerly Utilized Sites Remedial Action Program,
Revision 2, March 1987, and the Geotech Field Assessments Procedures Manual.
The radiological survey of this property included gamma scans of the entire ground
surface (interior and exterior), samples of exterior soil, and evaluation of historical data
when present. An assessment was also performed on this property to determine the areal
and vertical extent of the contamination. The results of the radiological assessment are
shown in the Appendix A.
2.0 Summary of Recommendation
DOE ID Number MS-01073, located at 381 South First West Street, is recommended for
inclusion based on the following:
• Radium-226 concentration exceeds the 'Hot Spot' guideline.
3.0 Results and Conclusions
This property was split off from DOE ID number MS-00233-MR. The interior
background exposure rate for the mobile home is 12 uR/h; the exterior background
gamma exposure rate is 14 pR/h. Two regions of exterior elevated gamma exposure
rates were found and labeled Regions E3 and E4 (Figure la, Appendix A).
The soil samples collected from Regions E3 and E4 combined (Location Numbers R6,
R7, and R8, Figure la, Appendix A) yielded a net area-weighted average radium-226
concentration of 3.7 pCi/g. This value is less than the Environmental Protection Agency
guideline of 5.0 pCi/g above background for surface soil averaged over 100 m2. The
soil samples collected from Region E3 resulted in an average gross radium-226
concentration of 27.2 pCi/g. This value is greater than the 'Hot Spot' guideline of
15.2 pCi/g for a 14.4 m2 area. The gross radium-226 concentration results for the soil
samples and in-situ measurements are shown in Table lb, Appendix A.

Executive Summary
1.0 Introduction
The following properties, consisting of residences in Monticello, Utah, were assessed by
RUST Geotech Inc. (Geotech)
• MS-O0233-MR, located at 96 West Fourth South Street
• MS-O1073-RS, located at 381 South First West Street
Following procedures described in the Field Assessments Procedures Manual, data were
collected on these properties and assessed to estimate the location of residual radioactive
material in excess of the Environmental Protection Agency (EPA) 'Standards for
Remedial Action at Inactive Uranium Processing Sites' (40 CFR 192).
1.1 DOE ID number MS-00233-MR
This property has been included for remedial action by the U.S. Department of
Energy on the basis of Oak Ridge National Laboratory's identification of excess
radium-226 concentration in the land area, radium-226 concentration exceeding the
'Hot Spot' guideline, and excess gamma exposure rate in the occupied or habitable
building.

This property is recommended for inclusion by Geotech on the basis of radium-226
concentration exceeding the 'Hot Spot' guideline.
2.0 Gamma Exposure-Rate Surveys
2.1 Exterior

The area backgrounds are 14 pR/h and 2.0 pCi/g. The high exterior gamma
exposure rate is 80 pR/h. Areas of elevated exterior gamma exposure rates
are shown in Figure la.

The area backgrounds are 14 uR/h and 1.0 pCi/g. The high exterior gamma
exposure rate is 32 pR/h. Areas of elevated exterior gamma exposure rates
ans shown in Figure la.
2.2 Interior

T&e interior background, as determined by Geotech, is 14 pR/h. The included
area averages 24 pR/h, and the high interior gamma exposure rate is 31pR/h.
Areas of elevated interior gamma exposure rates are shown in Figure lb.
2.2.2 DOE ID number MS-01073-RS
The interior background, as determined by Geotech, is 12 pR/h. The high
interior gamma exposure rate is 13 pR/h; there is no area exceeding the EPA
guidelines.
3.0 Radon/Radon Decay-Product Concentration (RDC)

An alpha-track measurement was collected by Geotech. The result of the 121-day
sample is as follows:

radium concentration in the contaminated areas is 177.5 pCi/g.

5.0 Remedial Action Recommendations
5.1 Exterior
5.1.1 DOE ID number MS-00233-MR
Exterior Deposit A (Figure 2a) should be removed and the appropriate cover
material replaced.
This property contains a deposit of uranium ore that exceeds 137.0 pCi/g
radium-226. The U.S. Department of Transportation requirements for Part 'B'
properties must be followed during remedial action.
5.1.2 DOE ID number MS-01073-RS
Exterior Deposits B and C (Figure 2a) should be removed and the appropriate
cover material replaced, if included by the U.S. Department of Energy.
This property contains deposits of uranium ore that are less than 137.0 pCi/g
radium-226. The U.S. Department of Transportation requirements for Part 'A'
properties must be followed during remedial action.
5.2 Interior
5.2.1 DOE ID number MS-00233-MR
Interior Deposit D (Figure 2b) should be remediated as follows:
Remove the brick walls and replace with new material.

Underground utility lines located during the assessment are shown on the drawings.
The exterior elevated readings on the north side of the one story frame house (Figure la) are
due to 'shine' from the adjacent interior contaminated brick walls.
Contamination in Deposit B (Figure 2a) appears to extend beneath the 3-inch-thick concrete
Interior contamination on DOE ID number MS-00233-MR (Deposit D, Figure 2b) is
associated with the 8-foot-high brick walls. A sample of the stucco, brick, and mortar were
obtained from the contaminated interior walls. The OCS results are as follows:
The results show the brick used in construction of the walls is contarninated.
There is no spillover to adjacent properties, as determined by a scintillometer scan.
