Subject: Notice of Source Deactivation for Levan Town Water System; UTAH12001

This letter serves as notification that the Division of Drinking Water (the Division) has changed
the activity status of the following source from “Active” to “Inactive”:
• Levan Park Well (identified as WS007)
The listed source is now considered inactive. Please contact David Kruse at dbkruse@utah.gov or
385-566-7789 prior to placing this source back online.
This decision was made after Levan Town Water System notified the Division on April 18, 2024,
that the Levan Park Well (WS007) was inactive during the first quarter of 2024. It was shared that
the source is planned to remain offline until a project is complete which will pipe the Levan Park
Well (WS007) to combine with the town’s springs represented at Sampling Station – 01 02 03 04
05 (identified as SS262).
Once this project is complete, all source samples shall be collected at the Sampling Station – 01
02 03 04 05 (SS262) during times when the Levan Park Well (WS007) and each of the town’s
springs are active and are represented in the sample.
Should the Levan Park Well (WS007) need to be utilized prior to the completion of this project,
please collect a Radionuclide sample from the Levan Park Well (WS007) and contact David
Kruse at dbkruse@utah.gov or 385-566-7789.
