Dear Wayne Gudgell:The Division of Drinking Water (the Division) received the plans and specifications for the proposed Pine Valley Highway Water Line Extension Project from Steven M.
Kamlowsky, Brown Consulting Engineers, P.C., on May 17, 2024. Written review comments were provided to you and your consultant on May 17, 2024. The Division received the revised plans
and specifications on June 3, 2024. This project has met the conditions for receiving a plan approval.
Our understanding of the project is to extend a main water line from the corner of Mountain View Drive, east along the Pine Valley Highway, to provide water service to a new church that
is planned to be built. The water line extension will be approximately 3,400 linear feet of 8-inch diameter PVC C-900 pipe, including fire hydrants, isolation valves about every 800
feet, and appurtenant fittings. A Hydraulic Modeling Certification was also submitted by Nathan Wallentine, P.E., Sunrise Engineering, who maintains a water model of the Dixie Deer
SSD water system, certifying that pressures meet the minimum requirements of the Division’s rules.
We have completed our review of the plans and specifications, stamped, and signed bySteven M. Kamlowsky, P.E., dated June 3, 2024, and found they basically comply with the applicable
portions of Utah’s Administrative Rules for Public Drinking Water Systems in R309. On this basis, the plans for Pine Valley Highway Water Line Extension are hereby approved.
This approval pertains to construction only. An Operating Permit must be obtained from the Director before this water line may be put in service. A checklist outlining the items required
for operating permit issuance is enclosed for your information.Approvals or permits from the local authority or the county may be necessary before beginning construction of this project.
As the project proceeds, notice of any changes in the approved design, as well as any change affecting the quantity or quality of the delivered water, must be submitted to the Division.
We may also conduct interim and final inspections of this project. Please notify us when actual construction begins so that these inspections can be scheduled.
This approval must be renewed if construction has not begun or if substantial equipment has not been ordered within one year of the date of this letter.
