Dear Jason Worwood:
The Division of Drinking Water (the Division) received a request for an Operating Permit for the
Levan Town Distribution System Improvement Project from your consultant, Tyler Faddis with
Jones and DeMille Engineering on June 7, 2024. The Division issued Plan Approval for this
project on June 21, 2023. Division staff conducted an onsite final inspection of this project on
June 4, 2024.
Our understanding of the project is that approximately 25,000 feet of existing distribution pipe has
been replaced with new 8-inch diameter PVC piping. Approximately 4,500 feet of new 8-inch
diameter PVC well transmission pipeline from the existing Park Well has also been installed. The
new distribution improvements are part of the existing distribution system and will be identified as
DS001 in the Division’s database
We have received the following information for the Levan Town Distribution System
Improvement Project:
1. Certification of Rule Conformance with plan approval conditions by a professional
engineer
2. Satisfactory bacteriological results as evidence of proper disinfection and flushing.
We have determined that all conditions for operating permit issuance have been met. On this basis,
an Operating Permit for the Distribution System Improvements is hereby issued as
constituted by this letter. You may now place the new distribution lines in service in your water
system.
