Dear Paul Garvey:
The Division of Drinking Water (the Division) received a request for an Operating Permit for the
350K Gallon Storage Tank project from your consultant, Tyson Jewkes with Ensign Engineering
on June 18, 2024. The Division issued Plan Approval for this project on October 17, 2022. DEQ
Central District Engineer, John Chartier, conducted an onsite final inspection of this project on
February 20, 2024.
Our understanding of the project is that a new 350,000-gallon concrete tank (ST003) has been
constructed adjacent to the two existing tanks, the East Tank (ST001) and the West Tank (ST002).
The new 350K Gallon Tank has been constructed at the same elevation as the existing tanks and
all three tanks share a common discharge point for their respective drain and overflow pipelines.
This project also included the installation of approximately 585 feet of 8-inch diameter PVC pipe
in the distribution system near 100 South and Main Street. This section of pipe included
reconnecting existing service connections and fire hydrants as well as new 8-inch gate valves. The
new 350K Gallon Tank is identified as ST003 in the Division’s database.
We have received the following information for the 350K Gallon Storage Tank(ST003) Project:
1. Certification of Rule Conformance with plan approval conditions by a professional
engineer.
2. Satisfactory bacteriological results as evidence of proper disinfection and flushing.
3. Evidence of O&M manual delivery.

We have determined that all conditions for operating permit issuance have been met. On this basis,
an Operating Permit for the 350K Gallon Tank Project is hereby issued as constituted by
this letter. You may now place the 350K Gallon Tank in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
