Dear Mr. Brown:
The files for these two releases have been reviewed by the Division of Environmental
Response and Remediation (DERR) project manager who has recommended that no further
corrective action be taken at this time. This no further action applies only to the specific Releases
Sites OFQ and OFY. This recommendation is based on the information contained in the file for the
release. That information was supplied by you, your consultant, or another representative.
The information submitted indicates that any detectable petroleum contamination from these
releases is not a threat to human health or the environment according to Utah Admin. Code Rule
R311-211. In the future, if other evidence indicates the presence of contamination from these
releases which may cause a threat, further corrective action may be required.

SUBJECT: No Further Action: Surface Spill, Aboveground Petroleum Storage Tank Release
Good to Go Store #419, located at 1010 North Main Street, Nephi, Utah
Facility Identification No. 2000655, Release Site OFY
I have reviewed the information for the above-referenced release, which includes information provided by the
facility owner. My recommendation that no further action is required for the release is based on the review
of the information contained in the case file, after due consideration of the corrective action clean-up standards
policy (Utah Admin. Code R311-211.
Case file information justifying no further action for the release includes the following:
1) 502-520 gallons of gasoline were released into, and some flowed out of, the APST containment and
onto asphalt outside containment on October 29, 2022 when the fuel delivery driver was
simultaneously refueling 2 APSTs and one overflowed. Some portion of the released product breached
the APST containment and entered the on-site, closed French drain system and was contained therein.
2) EnviroCare was dispatched the same day by the fuel delivery company. EnviroCare cleaned up all
released product, cleaning surface areas and using a jet rod truck to empty, flush and clean the French
drain system. No product left the site, nor reached soil or groundwater on-site.
3) EnviroCare disposed of 5.23 tons of water/fuel & dirt/debris from cleaning the drain system/vaults.
No Further Action is recommended for the release. Release data indicate no significant petroleum impacts
exist or remain on-site and do not appear to constitute a current or potential threat to human health or the
environment. This recommendation for close-out is based on the condition that if future evidence indicates
the existence of contamination at or emanating from this site, additional investigation and corrective action
may be required. A No Further Action letter will be sent to the responsible party.
