In accordance wittr Operatiag Permit Provision I.S.2.b, Utah Adrnhistrative Code (UAC)
R307-415-643XcXii) the following rcport is zubmitted:
I. List the date the deviation occurd: 05117n023
2. Permit conditiong forwhich dcvietion occurred: II.B.a.a.l(AXi)
3. Describe the devirtion that mcurred: The stack test for PMl0 emissions from
the number 4 kiln should have been done by 45rc3n023.
4. Deocribe lhe probable cauge of the deviation: The stack test was scheduled for
MlDnlB. On that day, the testing compafly was set up and ready but due to the cold
and wet winter weather conditions the raw shale was hangrng up in the feed tank and
would not {bed consistently into the kiln. Affer spending most of the day trying to get the
material to feed from the tank, the stack test was rcscheduled for slfin0B, Although
conditions had improved by 511712023 there were still some problems getting the raw
shale to feed to the kiln as consistently as neded for the stack test.
5. De*crlbe rny correclinte *cliow or preveative merlures takca: ThE kila operators
will continue to rnonitor the feed to tfu kiln and continue working to maintain a
consistent flow of material. The process should get back to normal as the weather gets
warmer. The stack test has been scheduled for the week of M10512023. The
representative from the Division ofAir Quality has been notified of the new date.
