The Division issued Plan Approval for this project on July 25, 2023. Our understanding of the project was the construction of a 750,000-gallon concrete storage tank (identified as ST004 in the Division’s database). The new storage tank will be supplied water by the Stewart Ranches Well (WS005) and will feed into the existing storage tanks (ST001-ST003). We have received the following information for Stewart Ranches Tank (ST004): 1. Certification of Rule Conformance with plan approval conditions by a professional engineer. 2. As-built or record drawings. 3. Satisfactory bacteriological results as evidence of proper disinfection and flushing. 4. Evidence of O&M manual delivery. We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for Stewart Ranches Tank (ST004) is hereby issued as constituted by this letter. You may now place the storage tank in service in your water system. Please maintain a copy of this letter with your permanent records for future reference. 195 North 1950 West • Salt Lake City, UT Mailing Address: P.O.
