The Division issued Plan Approval for this project on September 13, 2022. Division staff conducted an onsite final inspection of this project on May 15, 2024. Our understanding of the project is the removal and reconstruction of River Height City’s Lower Well (WS001) facility. The project included the demolition of the existing facilities, site excavation, grading and preparation, and construction of the well house. A gas chlorination addition (TP005) was built for chlorine residual maintenance only. Additional improvements included improvements to the facility telemetry, pump-to-waste, standby generator, and general site safety and security. We have received the following information for Lower Well (WS001) and Lower Well Chlorinator (TP005): 1. Satisfactory bacteriological results as evidence of proper disinfection and flushing. 2. Evidence of O&M manual delivery. We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for Lower Well (WS001) and Lower Well Chlorinator (TP005) is hereby issued as constituted by this letter. You may now place Lower Well (WS001) and Lower Well Chlorinator (TP005) in service in your water system. 195 North 1950 West • Salt Lake City, UT Mailing Address: P.O.
