Dear Mr. Wright,

This letter is being submitted as supplemental information to the official operating permit request
submitted by Northern Engineering in 2011 for the New Cook Well WS004.

In 2011 when the operating permit was requested for the New Cook Well the New Cook Well did
not have concurrence for source protection zones. The DWSP plans have been updated and the
Washington County Source Protection Ordinance has been applied to the New Cook Well. DDW
has drafted a concurrence letter for source protection zones for the New Cook Well.

Current Equipment for the New Cook Well:
Pump:
15 HP, Gould submersible pump
Model 65L
Installation date - 8.8.23
Current Pumping Rate – 70 gpm
