On February 20, 2024, and February 28, 2024, the Division of Waste Management and Radiation Control (Division) received your submission of results and applicable invoices for the Supplemental
Environmental Project (SEP) per the 2020 Stipulation and Consent Order (SCO) Number 2106050.
Your submission documented a total cost of $20,594.08. The amount to be credited per SCO #2105060 was $21,403.00. In order to close out this SEP, the Division hereby requests a cash
payment of $808.92 be made within 30 days of the date of this letter.
Payment shall be made to the State of Utah, Department of Environmental Quality, c/o
Director, Division of Waste Management and Radiation control, P.O. Box 144880, Salt Lake City, Utah 84114-4880.
