Dear Ms. Christoffersen:
This letter serves as a Notice of Transfer of Interest regarding that certain EPA ID
Number UTUR000013581 ("ID Number") issued to US Oil Sands, Inc. Attached please find an
executed RCRA Subtitle C Site Identification Form. The transfer will be effective upon closing
of that certain Amended and Restated Asset Purchase and Sale Agreement dated February 5,
2018, between FTI Consulting Canada, Inc. as Court Appointed Receiver and USO (Utah) LLC
("Purchaser"). The ID Number will be assigned to USO (Utah) LLC, a Delaware limited liability
company, whose address is 610 Broadway, 6th Floor, New York, NY 10012.
A certified copy of the Order issued by the United States Bankruptcy Court, Case No.
2:2017-bk-29716, authorizing the sale of the assets of US Oil Sands (Utah), Inc. to USO (Utah)
LLC was recorded on May 25, 2018 as Entry No. 2018003866, Book 1567, Pages 447-467, in
Uintah County, Utah and as Entry No. 529874, Book 868, Page 905, in Grand County, Utah.
