SITE VISIT NARRATIVE
On May 1,2018, representatives of the Division of Waste Management and Radiation Control
(DWMRC) and Attorney's General office met with Dale Urban, Site Assessment Section
Manager with DEQ's Division of Environmental Response and Remediation (DERR). The
purpose of the meeting was to inform Mr. Urban of the waste conditions at the Jensen property
located at 6594 Parkway Blvd., West Valley City, Utah in order to see if the conditions would
merit an emergency cleanup by EPA. Mr. Urban indicated that the conditions at the site
appeared to meet the criteria for an emergency cleanup. He also indicated that Marty McComb,
EPA's Region 8 On-Scene Coordinator, would be in Salt Lake City on May 2 and3,2018 and
hoped that he would be able to visit the Jensen property during his time in Utah.
On the morning of May 2,2018, Mr. Costomiris phoned Carrie Jensen, the resident of the
property and explained to her that Dale Urban of DERR, Marty McComb (EPA) and Dave
Larsen and Ed Costomiris (both of DWMRC) would like to visit her property. She said that that
would be fine. She explained that she would not be home when we got there as she was helping
a neighbor with a daycare for children located across the street. She provided verbal access to
her property in her absence. The purpose of the site visit was to provide a tour of the property to
Dale Urban and Marty McComb in order for them to assess the waste conditions on the property.
Based on their assessment, a decision will be made regarding the remediation of the site.
On May 3,2018, Mr. McComb and Costomiris returned to the Jensen Property so that Mr.
McComb could meet Mrs. Jensen and so that he could obtain an access form signed by Mrs.
Jensen. The access form addressed access to the property in order to determine the extent of
contamination and for access to remediate the property. While at the property, Mr. McComb
explained to Mrs. Jensen what the cleanup would involve. That included scooping and shipping
the spent blast media to an approved facility and the bulking of materials in drums and buckets
and having them sent offsite to an approved hazardous waste facility. Mrs. Jensen took the copy
ofthe access form for review by her attorney.
