Dear Mr. Stoddard: The Division of Waste Management and Radiation Control records indicate that your facility, located at
Freeport West D3, Clearfield, Utah, was identified as a Large Quantity Generator of hazardous waste in
Utah in 2017. In accordance with Utah Administrative Code R315-262-41, you are required to submit EPA Form 8700-13A/B (Biennial Report) by March 1st of every even-numbered year, for your generation activities during the previous year. The Division has tried to contact you numerous times by phone and email for submission of your report.

You are hereby requested to submit your completed Biennial Report for 2017 activities before June 19, 2018. Failure to provide the required information may result in additional enforcement action. If you have any questions, please call Carlee Christoffersen or Deborah Ng at [phone].
