Dear Mr. Roberts: The Division of Waste Management and Radiation Control records indicate that your facility, located at
11800 South U-111 Pipeline, Herriman, Utah, was identified as a Large Quantity Generator of
hazardous waste in Utah in 2017. Accordingly, you are required under Utah Administrative Code R315-262-41 to submit EPA Form 8700-13A/B (Biennial Report) by March 1st of every even-numbered year, for your generation activities during the previous year. The Division has tried to contact you numerous times by phone and email for submission of your report.
