Dear Mr. Buchanan: The Utah Division of Waste Management and Radiation Control records indicate that your facility,
located at 15988 Silver Pass Road, Eureka, Utah, was identified as a Large Quantity Generator of
hazardous waste in Utah in 2017 and is required under Utah Administrative Code R315-262-41 to submit EPA Form 8700-13A/B (Biennial Report) by March 1st of every even-numbered year, for your generation activities during the previous year. The Division has tried to contact you numerous times by phone and email for submission of your report.
