The source of the NAPL is believed to have been a crack in the nearby oily-water process sewer. The crack has been repaired and efforts to remove the NAPL from the well are ongoing. APPENDIX Boring Logs with Piezometer Completion Details and Laboratory Analytical Results ii Chevron Products Company and EMC LNAPL Release Investigation Salt Lake Refinery Summary Report May 10, 2018 CHEVRON SALT PRODUCTS COMPANY AND EMC CHEVRON SALT LAKE REFINERY WELL S-18A LIGHT NON-AQUEOUS PHASE LIQUID INVESTIGATION SUMMARY REPORT CHAPTER ONE INTRODUCTION This report presents the results of an investigation initiated in response to the June 2017 measurement of Light Non-Aqueous Phase Liquids, ("LNAPL") at Well S-18A, in the Southwest Tank Farm at the Salt Lake Refinery ("Refinery") in 2017. Figure 1-1 shows the operating portion of the Refinery with outlines of the areas that were investigated. 1-1 Chevron Products Company and EMC LNAPL Release Investigation Salt Lake Refinery Summary Report May 10, 2018 CHAPTER TWO FIELD INVESTIGATION After the June discovery of LNAPL in S-18A, Chevron directed EarthFax to purge the well of fluids and establish whether the LNAPL thickness would dissipate. That effort was conducted on June 28, 2017 using a vacuum truck. The LNAPL thicknesses on the 28th were measured prior to purging, after each purging event and upon completion after allowing time for fluid levels to equilibrate. The end result of this effort is that the LNAPL thickness was 0.46 feet greater than before the purge. Considering the 2-inch well casing diameter, the fluid column height in the well, aquifer characteristics and the 35 barrels purged, it was very apparent that there must be a nearby source of LNAPL that was in direct communication with the well. Based on the results of the purge event, an excavation was made on June 30, 2017 adjacent to the double vault just 9-feet west of well S-18A (Figure 2-1). The trench exposed two subsurface conditions that explain the high volume of water purged through the well and the increase in LNAPL as the well was purged. Specifically, • Several cubic yards of coarse, two to three-inch diameter angular gravel that was installed to support the pre-cast sewer vault extended six to twelve feet to the north and east from the vault. 2-1 Chevron Products Company and EMC LNAPL Release Investigation Salt Lake Refinery Summary Report May 10, 2018 • It was visually apparent that this gravel was in direct communication with the sand pack around the slotted casing in S-18A. Chevron had EarthFax conduct a shallow aquifer investigation which included installation of several small diameter borings completed with one-inch piezometers in the vicinity of S-18A and the excavation around the vault (Figure 1-1). A total of six temporary piezometers were installed during the investigation. The investigation methods, sampling procedures and post-installation LNAPL measurement procedures are described below. EarthFax mobilized on August 17, 2017 with the drilling contractor. The Direct Push drill rig began by advancing boring DP-1 in the area north of the referenced excavation along 300 West Street. Additional borings LCO-1, 2 and 3 were advanced to the south in the immediate area of well S-18A, the vault and the storm sewer line (Figure 2-1). The last two borings, identified as DP-88 and DP-90 were installed in the North Tank Farm. Monitoring During Boring Installation. All borings were installed following conventional environmental methods. This included measuring collected soil samples using a Photo- Ionization Detector ("PID") to measure total volatile organic vapors ("VOCs"). Elevated PID readings suggest the potential presence of hydrocarbons. Detailed logs of each boring are provided in the appendix to this report. Soil Sample Collection. Soil samples were collected into laboratory supplied jars from the depth intervals noted on the logs. After reviewing the logs and PID measurements for each boring, the sample LCO-2 at the 4.6 — 5.6 foot depth interval was selected for laboratory analysis. The sample was delivered under chain-of-custody to American West Analytical Laboratory ("AWAL") in Salt Lake City on the day of collection. The requested analysis was for benzene, ethylbenzene, toluene, xylenes and naphthalene ("BTEX-N") with gasoline range organics ("GRO") using EPA Method 8260 and diesel range organics ("DRO") using EPA Method 8105. Piezometer Completion. Each boring was completed as a piezometer to allow future measurements of both water level and LNAPL. Completion details are presented on the boring logs in the appendix. After installation, the piezometers were purged of approximately three casing volumes of groundwater and left for several hours to allow the groundwater elevation and LNAPL to equilibrate. 2-2 Chevron Products Company and EMC LNAPL Release Investigation Salt Lake Refinery Summary Report May 10, 2018 Water Level and LNAPL Measurements. Late on August 17th and again on the 22nd, each location was measured using an interface probe certified to detect thicknesses of 0.0312-inches (0.8 mm) or greater. No LNAPL was detected in any of the piezometers. Collection of Groundwater Quality Samples. After measuring the depth to the groundwater surface at each location, a dedicated bailer was used to collect a groundwater sample from piezometer LCO-3. The sample was delivered to AWAL with the soil sample from LCO-2 with a request to analyze for the same methods specified for the soil sample. To monitor the effectiveness of the Stormwater Sewer repair and removal of LNAPL from the subsurface, a measurement was made in well S-18A on December 5, 2017. At that time, 1.18-feet of LNAPL was measured on the groundwater surface in the well. On April 23, 2018, 1.00-feet of LNAPL was measured in the well. During the first quarter well S-18 was purged using a vacuum truck to remove LNAPL present in the well. Additional action will be taken and fluid levels will be monitored to see if the LNAPL attenuates. 2-3 Chevron Products Company and EMC LNAPL Release Investigation Salt Lake Refinery Summary Report May 10, 2018 CHAPTER THREE RESULTS AND CONCLUSIONS This chapter addresses the observations made during the advancement of the borings, subsequent LNAPL measurements for both events and the analytical results of the soil and groundwater samples collected from LCO-2 and 3, respectively. 3.1 OBSERVATIONS DURING BORING ADVANCEMENT As discussed in Chapter Two, borings were installed and completed as temporary piezometers at the locations shown on Figure 2-1. From field observations and the logs generated for each boring, the following general statements can be made. • In borings DP-1, LCO-1, 2 and 3 in the area around the exposed storm sewer and well S-18A, there were no indicators of hydrocarbons observed in the soil, including no detection of VOCs by the PID. There were elevated vapors in both borings at the North Tank Farm. However, those readings are consistent with other samples in that tank farm. . In the area around the storm sewer there were no organic vapors measured emanating from the completed borings at the ground surface prior to installation of the PVC casing, sand pack and bentonite seal. • There was no measureable LNAPL in the piezometers when measured on August 17 and 22, 2017 3.2 SOIL AND GROUNDWATER SAMPLE ANALYSES As noted above, the soil and groundwater samples were analyzed by AWAL. The laboratory report is provided in the appendix to this report. In summary: 3-1 Chevron Products Company and EMC LNAPL Release Investigation Salt Lake Refinery Summary Report May 10, 2018 3.2.1 Soil Sample LCO-2, 4.6' — 5.6' - Only Diesel Range Organics was detected at 25.2 mg/kg — dry. This concentration in soil from the shallow horizons at the Refinery does not suggest a release to or from the groundwater. 3.2.2 Groundwater Sample LCO-3 - There were no detections of volatile organics in the groundwater sample. 3.3 SEWER LINE REPAIR AND BLOCKAGE REMOVAL 3.3.1 Isolation of LNAPL in Gravels Near S-18A Based on the field observations and laboratory results of samples collected during this investigation, there was no indication that the LNAPL discovery in the storm sewer was contributing to or influenced by pre-existing contamination of the groundwater in the area near well S-18A and in the North Tank Farm. The upper soil horizons logged in the August 2017 borings show a very uniform distribution of fine-grained silty and clayey soils with occasional traces of fine sand making up the stratigraphic profile. Looking at the log for S-18A, which is only 4-6 feet from the storm sewer trench and vault presents an entirely different profile. The stratigraphy in the upper six- feet of well S-18A consists of silty sand with gravel, grading to medium dense silty sand to about twelve-feet. It is not until below twelve-feet that fine-grained soil appears on the log. The silty- sands, sands and gravels in S-18A have much higher hydraulic conductivity, allowing the LNAPL to travel through the subsurface on the groundwater at a much faster rate. The coarse sands and gravels observed in the excavation by the sewer vault adjacent to S-18A create a direct link with the sand pack around the slotted casing in S-18A. This continuity of coarse grained, highly permeable material explains how the LNAPL entered S-18A. 3.3.2 Source of LNAPL Several days after the subsurface investigation was completed, Chevron personnel extended the excavation around the storm sewer adjacent to S-18A to the north exposing a horizontal crack in the corrugated metal pipe. The flow of LNAPL from the sewer into the 3-2 Chevron Products Company and EMC LNAPL Release Investigation Salt Lake Refinery Summary Report May 10, 2018 coarse gravel towards the sand pack around S-18A was visible in the open excavation. The impacted area was limited because highly permeable gravels around the vault and the sewer line were contained within a small basin excavated into the low permeability fine grained soils when the new vault was installed in 2002. The fine grained soils were encountered in continuous beds in borings LCO-1, 2 and 3. 3.3.3 Discovery of Plugged Sewer Line In late July 2017, after Chevron had completed repairs on the Storm Sewer by S-18A, they hydro-jetted a long section of the Oily Water Sewer that included the lines directly south of S-18A on 300 North Street. With those lines cleared, normal flow was re-established to the wastewater plant and the LNAPL that had backed up into the storm sewer adjacent to S-18A flowed normally. The approximate area of the plugged line is shown on Figure 2-1. 3.4 MONITORING PLAN FOR FLUID LEVELS IN S-18A Following purge events, fluid levels in S-18A will be measured after allowing 24-hours for fluids to equilibrate.
