We have noted each issue below with our plan to address them.
• A Letter to the local Hospital and Police notifying them of our facility and any emergency scenarios that
they should be aware of.
Response: The attached letters were sent on April 27, 2016. These letters include the primary hazards associated
with the hazardous materials and wastes on site.
Please let me know if there is anything we may have overlooked, or if you would like any additional information.
We would like to be sure we have addressed all of your concerns.
In addition, we respectfully request confirmation (via email, phone call or written correspondence) that our
location has resolved any concerns and have obtained "Returned to Compliance" for our records.

non-flammable gasses; aerosol products; corrosive liquids; unstable and water-reactive liquids; solid
oxidizers; irritants; chemicals classified as potentially causing other health hazards, and refrigerant
containing anhydrous ammonia. The hazardous materials at this facility are stored throughout the
warehouse and in the receiving area. This Costco Wholesale location is registered as a Small Quantity
Generator (SQG) of RCRA hazardous wastes.
Enclosed for your information is a copy of the site layout, which can be used to familiarize yourself
with the emergency egress routes available at the facility. To make emergency response
arrangements, or any required agreements, please use the contact information below or contact the
warehouse directly.
In addition, we've enclosed a declination form in the event you choose to decline to make emergency
