Subject: Ground Water Quality Discharge Permit - Condition Temporary
Settlement Monument A-11 Corrective Action Plan
Dear Mr. Anderson:
As was discussed in the 2017 Annual As-Built Report for the Class A West Embankment,
Clive Utah (submitted via CD17-0265), EnergySolutions herein notifies the Division of
Waste Management and Radiation Control that it is implementing a Corrective Action
Plan to excavate the temporary cover and waste in the immediate vicinity of Temporary
Settlement Monument A-11. Implementation of the Plan herein summarized is in an
effort to identify and correct the source of unusual swelling and cracking observed at
Monument A-11 (but not seen in adjacent Monuments A-10, A-12, AT-10 and AT-11).
1) DEMARCATE CORRECTION AREA: Quality Control Supervisor will
demark a circular area with radius of 20 feet centered over Monument A-11.
2) CONSTRUCT TEMPORARY STAGING AREA: 6 inches of additional
compacted clay will be placed over temporary cover as a Staging Area adjacent to
the Correction Area.
3) REMOVAL OF TEMPORARY COVER: Temporary Cover within the
Correction Area will be excavated to an approximate 1 foot depth and placed in
the Staging Area.
4) REMOVAL OF EXPOSED WASTE: Excavation will start at the Monument
and expand outward and downward, as needed. If an obvious source of the
settlement discrepancy is found, the waste will be excavated along with additional
surrounding waste to assure complete removal. If an obvious source is not
located, waste will be removed up to approximately 6 feet of previously disposed
waste within the Correction Area and placed in the Staging Area.
5) EXPOSED WASTE EVALUATION: Exposed waste will be photographed and
visually inspected by the Quality Control Supervisor and Director of Engineering
to identify possible sources of the abnormal settlement observed at Monument A-
11.
6) EXCAVATION BACKFILL: Clay will be placed within the excavated
Correction Area, compacted in 1 foot lifts to 95% of a standard proctor.

7) TEMPORARY COVER PLACEMENT: Temporary cover will be replaced
over the Correction Area, in accordance with Work Element — Temporary Cover
Placement and Monitoring accordance of Revision 28b of the LLRW and lle.(2)
Construction Quality Assurance and Quality Control Manual (CQA/QC Manual).
8) SETTLEMENT MONUMENT PLACEMENT: Temporary Settlement
Monument A-11 will be replaced and surveyed.
9) DISPOSAL OF EXCAVATED FILL AND WASTE: Excavated fill and waste
will be transferred to an active lift elsewhere within the Class A West
Embankment and disposed, in accordance with applicable CQA/QC Manual
specifications.
10) LIMITED AREA RELEASES: Following removal of the excavated fill and
waste, Radiation Safety Technicians will conduct a 100% walk over radiation
survey of the Staging and Correction Areas to confirm the absence of residual
surface contamination (in accordance with CL-RS-PR-115. Radiological
Surveys). Any residual surface contamination detected will be removed.
11) SETTLEMENT MONUMENT MONITORING: Monthly settlement
monitoring will be conducted for the replaced Monument A-11 through May
2019. Settlement data will be reported to the Division in the 2018 and 2019
Annual Class A West Embankment As-Built Reports.
12) DIFFERENTIAL SETTLEMENT EVALUATION: After May 1, 2019, the
Director of Engineering will evaluate the differential settlement measurements
taken since implementation of the Plan to assess its effectiveness. If the abnormal
differential settlement has been corrected, Phase II of the final Class A West
Cover Construction will proceed. If the abnormal settlement continues to be
observed at Monument A-11, a Revised Corrective Action Plan will be
implemented.
