RE: Compliance Evaluation Inspection UTD073000895 Dear Mr. Patitas:
On March 28, 2018, a representative from the Division of Waste Management and Radiation Control conducted a compliance evaluation inspection at your facility to determine compliance with the Utah Solid and Hazardous Waste Rules.
A review of our records indicates that Weber State University (WSU) filed its original notification to obtain an EPA Identification Number in 1986. Since that time, some additional notification requirements have gone into effect for university laboratories.
More specifically, in the case of WSU, eligible academic entities, i.e., universities with laboratories, that opt into or withdraw from managing laboratory hazardous wastes pursuant to R315-262-200 through 216 must now re-notify or amend their existing notification on EPA form 8700-12, 8700-13 a/b. 8700-23, Box # 12. This form can be found on the Division’s web page as discussed during the inspection or filled out online
