RE: Compliance Evaluation Inspection
Dear Ms. Evans:

On May 8, 2018, a representative of the Division of Waste Management and Radiation Control inspected
your recycling facility located at 925 Bending River Road. The inspector met with your Safety Manager, Frank Ravarino, to look at the facility and to review available documents. The observations made during the
inspection indicated that you are operating your facility in accordance with the approved Plan of Operation.

Since neither you nor Tracy Evans, were available to determine the location of some documentation during the inspection, the Division will follow up with a phone call or visit to review addition documentation.
A review of our records indicates that there have not been any updates to the closure costs since the
application for the Plan of Operations was approved. A review of the Plan of Operation indicated that the
estimated closure cost was $4,140 in 2011. The Division requests that estimate for closure of your facility be updated and that you provide a mechanism for financial assurance that meets the requirement of the Solid
Waste Rules, R315-309 of the Utah Administrative Code.
