Dear Mr. Adams:
Your application for registration as a waste tire transporter is approved. Revolution Tire Recycling LLC has
been assigned the solid waste tire transporter number TRl813. This number should be used on all future
correspondence sent to the Division regarding transporter of waste tires in the state of Utah.
This registration as a waste tire transporter will be valid until December 31, 2018 by which time a new
registration must be filed with the Division.
This approval is required in order to be in compliance with the requirements for the ffansporter of waste tires
in the state of Utah. Maintenance of a valid registration and compliance with all applicable requirements for
a waste tire transporter does not relieve waste tire transporter of the responsibility to obtain and comply with
any other required local, state or federal approvals or permits. This registration may be revoked in
accordance with Utah Administrative Code R3 1 5-320-4( I 0).
