Dear Mr. Adams:
Your application for registration as a waste tire recycler is approved. Revolution Tire Recycling LLC has
been assigned the solid waste tire recycler number TRR1814. This number should be used otr all future
correspondence sent to the Division regarding recycling of waste tires in the state of Utah.
This registration as a waste tire recycler will be valid until December 31, 2018 by which time a newregistration must be filed with the Division.
This approval is required in order to be in compliance with requirements for the recycling of waste tires inthe state of Utah. Maintenance of a valid registration and compliance with all applicablJrequirements for a
waste tire recycler does not relieve waste tire recycler of the responsibility to obiiin and comply with any
other required local, state, or federal approvals or permits. This iegistration may be revoked in accordancewith Utah Administrative Code R3l5-320-5(9).
