Dear Mr. Jackson: The purpose of this letter is to notify you that, as a registered Utah Waste Tire Transporter, your
application to renew the registration for 2018 is past due. Your waste tire transporter registration
expired on December 31, 2017. A new registration must be filed with the Division of Waste Management and Radiation if you wish to continue as a waste tire transporter. Registration forms can be found on the Division’s web page at:
