Steves & Sons (Steves) operates a door-manufacturing facility in Brigham City, Utah (the Facility). On
February ).6, 2024, Steves was made aware of a notice of complaints of excessive dust resulting from the
Facility. This notice was forwarded to the Utah Division of Air Quality (UDAQ) from Region 8 of the United
States Environmental Protection Agency (EPA). This letter addresses the circumstances that led to this
notice and the means Steves is taking to avoid a recurrence.
After receipt of the notice, Steves conducted an internal investigation into the occurrence. It was
determined that the excess dust that led to the complaint was most likely caused by an isolated incident.
One of Steve's contract truck drivers did not put the dust-control tarp in its place over truck load before
leaving the Dust Collection Area at the Facility. The driver then proceeded to the north side of the Facility,
which is closest to the neighboring houses. After parking, the driver put the dust-control tarp over the load
However, the wind was blowing and had already carried fugitive dust towards the nearby houses. This
event is not representative of the Facility's standard operations. Typically. each driver is instructed to cover
their load before leaving the Dust Collection Area and drive to the south side of the Facility, away from the
nearby houses.
Steves has taken the following steps to avoid a recurrence. Dust Collection Area training is being developed
and will be given to all of Steves' maintenance personnel who interface with contract truck drivers. This will
enable maintenance personnel to convey the Dust Collection Area's operating protocol to all contract truck
drivers face-to-face. Signs will also be posted describing the protocol. This should prevent haul trucks from
leaving the Dust Collection Area before covenng their load with the dust-control tarp. It should also prevent
haul trucks from driving to the north side of the Facility. The necessity of regularly sweeping the Dust
Collection Area is being reiterated to facility personnel.

Hello Austin,
I am contacting you because you are listed as the point of contact on the Approval Order (AO) for the new Steves and
Sons Door Manufacturing Facility, issued on December 23, 2022, in Brigham City, Utah.
We have been forwarded complaints from Region 8 EPA in Denver asking us to investigate excessive dust complaints at
the aforementioned facility located at 1080 North Main Street, Brigham City, Utah.
Be advised that condition II.B.1.a of the AO (copy attached) requires the facility to control all visible emissions associated
with source operations and failure to do so are violations of the AO.
Additionally, AO condition I.8 requires an 18-month notification on the status of construction of the facility. Based on the
information in the complaints, operations have commenced at this facility but no notification documenting this status has
been submitted to the Utah Division of Air Quality.
Please provide a response to this email detailing what actions have been taken to address these potential violations of the
AO.
