Re: Notification of Ventilation Hole Replacement, the La Sal Mines Complex

Mr. Bird:

Energy Fuels Resources (USA), Inc. (“EFRI”) notified Division of Oil, Gas and Mining (the
“Division) that EFRI would be performing maintenance on Pandora 9 (the “Vent”) by letter dated
February 6, 2024 (Attachment A). The Vent is located within the La Sal Mine Complex (the
“Mine”) area as shown in Figure 1 of Attachment B. The Vent had experienced sloughing of
material into the hole opening, temporarily blocking off the Vent preventing air flow exchanges.
Vents are needed to provide fresh air exchanges for the safety of our employees working
underground in accordance with state and federal requirements.
A drill rig was contracted to complete the maintenance on the Vent as previously described.
While the rig was being set up to begin the maintenance, the concrete pad around the vent
collapsed rendering the Vent unusable. Therefore, EFRI is going to replace the Vent at this time.
The replacement Vent will be installed within the existing disturbance area using the contracted
drill rig. The Vent is located on lands managed by the United States Forest Services (“USFS”).
Therefore, the Vent will be constructed in accordance with the Division and USFS requirements.
The Vent will not intercept the D-aquifer; however, EFRI will be casing the Vent with this
installation. As the Vent is being replaced within the existing disturbance all baseline surveys
were previously completed and are part of the approved Environment Assessment.
There will be no increase in surface disturbance as the Vent is going in the same area of existing
disturbance. Likewise, no new access to the Vent is required as that is already established. The
Vent is also already bonded through the surety approved by the Division in letter dated April 5,
2023, and there will be no need to revise the bond amount as a result of this replacement.
Once constructed, the Vent will be operated and monitored according to the above-named
agencies’ requirements.
