Dear Mayor Noreen Johnson:

The Division of Drinking Water (the Division) received a request for an Operating Permit for the
Mountain Spring Transmission Pipeline Repair/Replacement from your consultant, Kelly
Chappell with Ensign Engineering on November 15, 2023. The Division issued Plan Approval for
this project on August 31, 2023.

Our understanding of the project is that a large section of the existing 4-inch diameter PVC pipe
that carries the mountain spring water to the storage tanks was damaged and has now been
replaced. The section of piping that has been replaced is immediately above the storage tanks and
chlorination building. This project replaced approximately 2,600 feet of the existing 4-inch
diameter PVC piping with 6-inch diameter HDPE DR 11 piping. The necessary valving was
installed in the new pipeline and a new turn-out was placed on this pipeline just above the existing
meter station.

We have received the following information for the Mountain Spring Transmission Pipeline
Repair/Replacement Project:

1. Certification of Rule Conformance with plan approval conditions by a professional
engineer.
2. As-built or record drawings.
3. Satisfactory bacteriological results as evidence of proper disinfection and flushing.

We have determined that all conditions for operating permit issuance have been met. On this
basis, an Operating Permit for the Mountain Spring Transmission Pipeline is hereby issued
as constituted by this letter. You may now place the new spring transmission pipeline in service
in your water system.

Please maintain a copy of this letter with your permanent records for future reference.
